Rozporządzenie Ministra Pracy i Polityki Socjalnej z dnia 7 maja 1999 r. zmieniające rozporządzenie w sprawie sposobu przeprowadzania wywiadu środowiskowego (rodzinnego), wzoru kwestionariusza wywiadu oraz oświadczenia o stanie majątkowym, rodzaju dokumentów wymaganych do przyznania renty socjalnej, a także wzoru legitymacji pracownika socjalnego.
