Uchwała Państwowej Komisji Wyborczej z dnia 12 sierpnia 1998 r. w sprawie sposobu zgłaszania kandydatów na członków komisji wyborczych, wzoru zgłoszenia oraz zasad powoływania terytorialnych komisji wyborczych i obwodowych komisji wyborczych w wyborach do rad gmin, rad powiatów i sejmików województw.
