Rozporządzenie Ministra Spraw Wewnętrznych z dnia 12 maja 1995 r. zmieniające rozporządzenie w sprawie tworzenia, organizacji i kontroli zakładów opieki zdrowotnej utworzonych przez Ministra Spraw Wewnętrznych, rodzajów dokumentacji medycznej, sposobów prowadzenia i szczegółowych warunków jej udostępniania w tych zakładach.
