Regulation of the Minister of Labour and Social Policy of July 27, 1999 on the detailed principles and procedure for issuing medical certificates, the form of a medical certificate, and a medical certificate issued as a result of a control by a certifying doctor of the Social Insurance Institution.
Rozporządzenie Ministra Pracy i Polityki Socjalnej z dnia 27 lipca 1999 r. w sprawie szczegółowych zasad i trybu wystawiania zaświadczeń lekarskich, wzoru zaświadczenia lekarskiego i zaświadczenia lekarskiego wydanego w wyniku kontroli lekarza orzecznika Zakładu Ubezpieczeń Społecznych.
