Rozporządzenie Ministra Pracy i Polityki Społecznej z dnia 1 października 2010 r. zmieniające rozporządzenie w sprawie szczegółowych zasad i trybu wystawiania zaświadczeń lekarskich, wzoru zaświadczenia lekarskiego i zaświadczenia lekarskiego wydanego w wyniku kontroli lekarza orzecznika Zakładu Ubezpieczeń Społecznych.
