Regulation of the Minister of Labour and Social Policy of August 2, 1999 on the determination of the forms of documents and other evidence necessary to establish the right to family, care, and upbringing allowances, and the detailed principles and procedure for paying these allowances.
Rozporządzenie Ministra Pracy i Polityki Socjalnej z dnia 2 sierpnia 1999 r. w sprawie określenia wzorów dokumentów i innych dowodów niezbędnych do ustalenia prawa do zasiłku rodzinnego, pielęgnacyjnego i wychowawczego oraz szczegółowych zasad i trybu wypłaty tych zasiłków.
