Regulation of the Minister of Labour and Social Policy of April 16, 2002, on determining the form of application for establishing entitlement to family, care, and upbringing benefits, other documents necessary for establishing the right to these benefits, and the detailed rules and procedure for their payment.
Rozporządzenie Ministra Pracy i Polityki Społecznej z dnia 16 kwietnia 2002 r. w sprawie określenia wzoru wniosku o ustalenie uprawnień do zasiłku rodzinnego, pielęgnacyjnego i wychowawczego, innych dowodów niezbędnych do ustalenia prawa do tych zasiłków oraz szczegółowych zasad i trybu ich wypłaty.
